PASSPORT RENEWAL (For Filipino Maids Only)

Please bring the maid and the following documents along:

1. 2 Original Standard Contract Authenticated by Philippine Embassy (to be signed by the employer and the FDW)
2. 2 Original copies of the Undertaking of Employer for Employment of FDW
3. OWWA Membership (to be signed by the FDW)
4. Philippine Passport Application Form
5. 1 Original Copy of S$2,000 Philippine Embassy Bond (to be signed by the Employer)
6. 1 copy of the FDW's valid Medical Insurance
7. FDW's Original Passport
8. 3 copies of FDW's Philippine Passport and Work Permit Card
9. 3 copies of maid's photos (passport size, royal blue background, applicant in light colored shirt with collar and sleeves)
10. A copy of Employer's NRIC / Passport for Expatriate

- Forms for numbers 1-4 are available in our office

FEES:
S$ 370 (new clients)
S$ 340 (existing clients)

** Please take note that the fees mentioned above does not include the $2000 Philippine Embassy Bond, which may need to be purchased separately with an Insurance Company, (estimated S$42.80)**

RENEWAL OF WORK PERMIT (For Filipino & Indonesian maids)

Please bring the following documents along:

1. S$5,000 Security Bond and S15,000 personal accident insurance (to be signed by the Employer)
2. Security Bond Form (to be signed by the Employer)
3. Renewal Form from MOM (to be signed by the Employer and FDW)
4. FDW's Original Passport
5. FDW's Work Permit Card

- Forms for numbers 1-2 are available in our office

FEES:
S$ 70 (new clients)
S$ 50 (existing clients)

** Please take note that the fees mentioned above does not include any Insurances or Banker's/Insurance Guarantee, which may need to be purchased separately **

3. OWWA membership certificate / card
4. 1 Copy of the Two-way Air Ticket
5. Original FDW's Philippine Passport and Work Permit Card
6. 1 Copy of Employer's NRIC / Passport for Expatriate

WORK PASS (E PASS / S PASS) APPLICATION

Please bring the maid and the following documents along:

1. Employment Pass / S Pass Application form
2. Authorisation Form
3. 1 Copy of Applicant's Passport
4. 1 Copy of Applicant's (FIN) Foreign Identification Number (If applicable)
5. 1 Copy of Applicant's old/new Malaysian Identification Number (If applicable)
6. 1 Copy of Applicant's Work Permit Card (If applicable)
7. Applicant's Educational Qualifications (Transcript of Records, Diploma & other Certificates)
8. Applicant's Work experience details and Testimonial Certificates from previous work/s
9. Applicant's spouse personal particulars (If accompanying spouse is a Singapore citizen / Permanent Resident / Employment Pass / S Pass or Work Permit holder)

- Forms for numbers 1-2 are available in our office

FEE: S$ 100

** Please take note that the fees mentioned above does not include application for Appeal and Work Pass Issuance that needs to apply online and pay MOM (S$30 per year of the validity of Pass).**
