Please bring the maid and the following documents along:

1. Philippines contract (to be signed by employer and FDW)
2. OWWA membership (to be signed by FDW)
3. $2000 Philippines Embassy bond (to be signed by FDW)
4. Philippines passport application
5. FDW's original passport
6. A copy of FDW's work permit card
7. 3 copies of maid's photos (passport size, white background, with collar)
8. A copy of employer's NRIC / passport for expatriate

* For no. 1-4, forms are available in our office.

** Please take note that the fees mentioned above does not include the $2000 Philippine Embassy Bond, which may need to be purchased separately with an Insurance Company, (estimated S$42.80)**
